Varies The Price of Truth
The Mirror of Truth grants the following properties to its wielder:
Revelation of Truth:
When a creature gazes into the mirror, they see one hidden truth about themselves, another person, or something they seek. The creature must make a DC 18 Wisdom saving throw.
- On a success, the mirror reveals valuable insight, such as a secret, the true nature of an object or person, or a critical piece of information.
- On a failure, the creature is overwhelmed by an influx of unwanted truths and disturbing visions, becoming stunned for 1 minute as their mind processes the deluge of information.
Insight of the Damned:
As an action, the wielder can use the mirror to target a creature within 60 feet. The target must make a DC 18 Wisdom saving throw.
- On a failed save, the wielder learns one of the following:
The creature’s greatest fear.
The creature’s most shameful secret.
A moment from their past that they would do anything to keep hidden.
The target also takes 4d6 psychic damage from the mental intrusion. This ability can be used a number of times equal to the wielder’s proficiency bonus, refreshing after a long rest.
Reflections of Fear:
Once per day, the wielder can manifest the target's deepest fears. As an action, the wielder targets all creatures within a 30-foot radius. Each creature must make a DC 18 Wisdom saving throw. On a failed save, they are frightened for 1 minute, seeing haunting illusions of their personal fears. A frightened creature can repeat the save at the end of each of its turns, ending the effect on a success.
Gaze of the Hidden Self:
By focusing on the mirror for 10 minutes, the wielder can reveal magical disguises, illusions, or invisibility within 60 feet. The mirror uncovers the true form of any creature or object cloaked by illusion magic, revealing the deception for 1 hour. This ability can be used once per long rest.
Enhanced Properties (Post-Attunement)
Once attuned, the Mirror of Truth unlocks its full power, granting the wielder the following additional benefits:
Whisper of the Past:
The wielder gains the ability to cast legend lore without material components. This ability can be used once per long rest.
Eyes of Absolute Clarity:
The wielder gains truesight (60 feet) while gazing into the mirror. This truesight can be maintained for up to 10 minutes. This ability can be used once per short rest.
Truth Seeker’s Will:
The wielder becomes immune to the charmed condition. If another creature attempts to lie or deceive the wielder within 30 feet, the wielder is immediately aware of the deception, though they do not automatically learn the truth.
Scrying’s Reflection:
The wielder can cast scrying without requiring material components. This ability can be used once per day, and the creature being scried on has disadvantage on the saving throw.
Special Power - Dark Revelation
Once per month, the wielder can invoke the Dark Revelation of the Mirror of Truth. The wielder may ask a question about a person, event, or situation, and the mirror will provide a vivid, tangible vision that answers the query. The answer is 100% true, but it will always reveal a darker or more tragic aspect of the truth.
- The wielder permanently loses 1 point of Wisdom as the mirror consumes part of their mind.
- The vision is filled with horror and suffering, potentially leading to emotional damage or mental strain.
- The wielder gains disadvantage on Wisdom (Insight) checks until their next long rest, as they struggle to process the grim truth they have seen.
Curses
Burden of Knowledge:
Each time the wielder extracts information from a creature or object using the mirror, they must make a DC 18 Wisdom saving throw. On a failed save, the wielder is haunted by fragments of dark knowledge, suffering disadvantage on Wisdom and Charisma checks until their next long rest. Failing this save three consecutive times causes them to become paranoid, believing that everyone is hiding something from them.
Soul’s Reflection:
If a creature is incapacitated or killed while gazing into the mirror, a fragment of their soul is trapped within it. The mirror whispers using the voices of these trapped souls, increasing over time. Each dawn, the wielder must make a DC 16 Wisdom saving throw. On a failed save, they suffer disadvantage on all Intelligence checks as the whispers cloud their mind.
Obsession with Truth:
The mirror compels its wielder to extract secrets. If the wielder goes 48 hours without extracting a secret, they suffer 5d10 psychic damage. Additionally, the mirror reappears in the wielder’s possession within 24 hours if discarded or lost.

When one gazes into it, they do not see a simple reflection but instead behold their deepest secrets, darkest fears, and truest self. Said to have been created by a long-forgotten entity obsessed with absolute truth, the Mirror of Truth slowly drives its wielder to obsession, compelling them to extract secrets from those around them. The mirror speaks in soft whispers, its voice reflecting the fragmented souls it has captured. Those who wield it walk the line between insight and insanity.
